End of life
In most cases, end of life support is required for clients who have been discharged by their hospital or care facility as they wish to die at home. Pulse Community Healthcare, on receiving a request to support a rapid discharge, is able to implement a rapid response care plan quickly to ensure a smooth transition between discharge and arrival home, while providing support for the client and their family.
Pulse Community Healthcare also provides palliative support to clients who are not receiving curative treatment but have not yet reached ‘end of life’ stage.
We’ll work alongside charities such as Macmillan Cancer Support as well as district nursing teams, and our staff members receive specific training to ensure clients receive the best possible end of life and palliative care. We also provide bereavement support.
